Information required on the entry form consists of your First Name, Last Name, Address (no P.O. Boxes), City, State, Zip, Daytime Telephone Number (including area code), Email Address, Date of Birth, and Gender (collectively “Entry Information”). Limit one (1) Online Entry per person/email address/calendar day. Online Entries by the same person in excess of this limit will be disqualified. Online Entries must be received by 11:59:59 p.m. EDT on July 1, 2005. Eligibility: This Sweepstakes is open only to legal residents of the 49 states of the U.S. (Florida is excluded) and the District of Columbia, who are physically located and residing therein, who are 21 years of age or older as of April 15, 2005. Residents of Florida are not eligible to enter this Sweepstakes. Approximate Retail Value (“ARV”) of the Grand Prize: $6,633. Actual value of air transportation and hotel will depend on point of departure, airfare fluctuations, and time of travel. Drawing and Notification of Winners: Potential winners will be randomly selected from all eligible Sweepstakes Entries received in a drawing held on or about July 15, 2005 (“Drawing Date”) by Sweepstakes Administrator, an independent judging organization. Potential Grand Prize winner and potential First Prize winner will be notified by phone (at the phone number provided on that entrant’s Sweepstakes Entry) within two (2) weeks of the Drawing Date. All other potential winners will be notified by mail within two (2) months of the Drawing Date. If a potential winner cannot be reached after a reasonable effort has been exerted, he/she is found to be ineligible, he/she cannot or does not comply with these Official Rules, and/or if his/her prize or prize notification is returned as undeliverable, such potential winner will be disqualified and, if applicable, an alternate potential winner of such prize will be selected at Sponsor’s sole discretion.
