Then, mail this completed Submission Release Form, along with your photograph(s) and/or VHS videotape (up to 5 minutes in length), or your written materials to:

I am hereby submitting to Woodridge Productions, Inc. ("Producer"), a VHS videotape and/or photographs(s) or written materials which contains my appearance or my writing, as described above ("Material") and in consideration of the opportunity to be considered for a possible segment for the television series "Ripley's Believe It Or Not!" (the "Series") and for other good and valuable consideration, I hereby agree as follows:

I understand that Producer, as a matter of company policy, will refuse to accept, consider or otherwise evaluate the Material unless this Submission Release Form is signed and returned with the Material. I also understand that no confidential relationship is established between Producer and me by my submission of the Material to Producer hereunder. I hereby request and give permission to Producer to evaluate my appearance and/or written materials contained in the Material in connection with the Series. I further agree that no obligation of any kind is assumed or may be implied against Producer by reason of Producer's evaluation of the Material, and that Producer is not obligated to use the Materials or any of the rights granted in this Submission Release Form, including, without limitation, the right to broadcast (in any media) my appearance or the written portions contributed by me encompassed in the Material ("Appearance"). I have retained at least one copy of the Material and acknowledge that Producer will not return the Material and shall not be responsible, financially or otherwise, for any loss or damage or destruction or other failure to return the Material.

I represent and warrant that all statements, remarks, personal identifying details and facts and all other information of any kind or nature contained in my Appearance or in the Material, are true, correct, and accurate and further, that I have not omitted or withheld information in connection with my submission of the Material for the purposes of misleading anyone or misrepresenting any facts or circumstances being presented on the Material. I further represent and warrant that I am the sole owner (except in the case of a duo performance or co-writings, in which I and my co-performer or co-writer are the sole owners) of the Material, that I have the exclusive right and authority to submit the Material to Producer upon the terms and conditions stated herein and that all of the elements of the Material are accurately described above. I agree to indemnify, defend and hold Producer harmless for any liabilities, losses, claims, demands, costs (including reasonable attorneys' fees), or expenses arising in connection with any breach or alleged breach of the foregoing. For the purposes of this Submission Release Form, the term "Producer" shall be deemed to include any and all subsidiary, affiliated and related entities and each of their respective employees, officers, directors, agents, representatives, licensees and assigns.

I irrevocably grant to Producer and its successors, assigns and licensees, all right, title and interest in and to my Appearance and the Material, including the copyright therein; the right to use my Appearance and any and all information, including biographical information and/or other materials I have supplied, as well as other information Producer may have received from other sources, in connection with the Series; to re-record my Appearance in any medium, now known or hereafter devised; to edit my Appearance in any manner in Producer's sole discretion; and to distribute, exhibit and exploit my Appearance on the Internet and in any and all media, by any method now known or hereafter devised, throughout the universe in perpetuity, including for advertising, promotion and merchandising of the Series.

I will not institute or support any claims or actions against Producer arising out of the use of my Appearance and I hereby release Producer from and against any and all claims, demands and causes of action of every kind which I may have now or in the future based upon or arising from my Appearance or my participation in the Series, or any exercise by Producer of its rights as conveyed in this Submission Release Form, including but not limited to any claims for defamation, violation of rights of privacy and/or publicity, intentional infliction of emotional distress, copyright infringement, and/or any other tort in connection therewith. I also recognize that Producer may have access to and/or may create or have created concepts, ideas or material which may be similar or identical to my Appearance in any and all respects. I agree that I will not be entitled to any compensation or credit because of the use of any such similar or identical concepts, ideas or material which may have been independently created by Producer or may have been received by Producer from any other independent source and further, I shall not have the right to seek an injunction or other equitable relief against Producer as a result thereof.

I agree that if my Material is selected as one of the segments for the Series and/or is posted on the "Ripley's Believe It Or Not" Web site, I will not use that fact in connection with any publicity, promotion or commercial advertising not undertaken by or under the control of Producer, except as pre-approved in writing by Producer. As of this date, I am not a candidate for any elected federal office and I will not be a candidate for any elected public office during the 2003/2004 production season.

The Terms and Conditions of this Submission Release Form shall be construed and interpreted pursuant to the laws of the State of California applicable to agreements entered into and fully performed in California. All disputes arising hereunder shall be resolved in a court of competent jurisdiction located in Los Angeles, California and the parties consent to the jurisdiction of such courts and agree to accept service of process by first class United States mail. The prevailing party in any such dispute shall be entitled to recover reasonable attorneys' fees. Any controversy, dispute or claim under, arising out of, in connection with or relating to this Agreement, including, but not limited to the negotiation, execution, interpretation, construction, coverage, scope, performance, breach, termination, validity or enforceability of this Agreement, shall be settled, at the request of either party, by reference to a judge in accordance with the procedures of §638 et seq. of the California Code of Civil Procedure, sitting without a jury.

I hereby acknowledge that I have read and understand the terms and conditions of this Submission Release Form and that no oral representations of any kind have been made to me, and that this Submission Release Form states our entire understanding with reference to the subject matter hereof. Any modification or waiver of any of the provisions of this Submission Release Form must be in writing and signed by both parties. I represent and warrant that I have the full right, power and authority to grant the rights herein. I understand that Producer is making substantial expenditures in reliance upon this Submission Release Form and that I may not revoke the rights I have granted herein. I hereby certify that I fully understand the meaning and effect of this Submission Release Form and that I intend to be legally bound hereby.

I represent and warrant that I am 18 years of age or older; if I am under the age of 18, my parent/legal guardian has approved the terms of this agreement where indicated below.

I hereby agree to and confirm the terms of this agreement on behalf of my child or ward, and I represent and warrant that I am legally authorized to do so.
