APPLICATION INSTRUCTIONS
1. Answer all questions and fill out the application as completely as possible.
2. Answer honestly and to the best of your ability.
3. You must be at least 18 years of age and be able to show verification that you have a legal right to work in the United States.
4. If you are selected as a participant, you agree to authorize producers to conduct background checks.
5. Please carefully read the terms and conditions on the last page of this application prior to clicking the submit button. Please submit the entire application completed and signed when finished.
6. This application will only be reviewed by the producers and the Casting Department of Worst-Case Scenario and the appropriate parties at Avoca Productions, Inc. and Sony Pictures Television.
7. Upload a recent photo of yourself with this application (in .jpg/.gif format only).
8. Please make a TWO MINUTE videotape of yourself, showing us that you are adventurous, outgoing and brave enough to be selected for Worst-Case Scenario.
9. Applications MUST be accompanied by a photograph and a videotape which meets the following restrictions -- Length: Maximum of two minutes. Format: VHS, VHS-C or mini-DV. We prefer VHS. To transfer other formats to VHS simply connect your camera to your VCR and record.
